Case Management | |

The goal of case management is to help clients maintain independence and control over their lives and the decisions that affect them. This is achieved by linking them to needed services. Coordination of services and continuity of care are entrusted to a case manager who completes an in-depth assessment of each client's unique situation. This assessment includes medical, social, psychological, and survival needs as well as a review of medical, personal, financial, family, and community resources. The clients' families are actively involved in the process and their needs for support and information are addressed as well. | |
Information and Referral | |

Provided by case managers and other staff, information and referrals are available to both clients and the general public using our extensive resource files, which include community services available to clients, up-to-date educational materials on treatment and medical information and information on community programs and service available to our clients and their families. | |
Education | |
|
Educational programs are provided by certified educators and persons with AIDS for schools, colleges, community, and civic groups. Up-to-date educational materials are also available. | |
Holiday Program | |
|
The holiday program provices winter clothing and gifts to children whose families are affected by HIV/AIDS, as well as an annual holiday dinner and party. | |
Subsidized Therapies | |
|
Strongest Link is a member of a consortium known as OASIS. Through a combined effort with other HIV service organizations, funding has been obtained to subsidize vital acupuncture treatments, mental health counseling, emergency relief, child care, home-delivered meals, and nutritional consultation. | |
Practi-Care | |
|
Our Practi-Care program offers transportation, subsidized child care, and an errand program for our clients' basic needs. | |
